Think of it as a "health home base." Instead of driving all over town to see different specialists or trying to coordinate your own physical therapy and prescriptions, Welbe PACE coordinates everything for you.

When I say "everything," I mean it. From your primary doctor and specialists to your dental care, vision, and even your transportation to get there, it’s all covered and managed by one dedicated team.

Do You Qualify? (The Zip Code Rule)

Because Welbe PACE is so hands-on, you do have to live within the service area of one of their centers. I specialize in the following areas:

General Eligibility Requirements:

  • You are 55 years of age or older.
  • You live in a Welbe PACE service area (zip code check!).
  • You meet the state’s requirement for nursing home level of care (but can live safely in the community with PACE support).
  • You have Medicare, Medi-Cal, or both. (If you have both, the program is typically $0 out of pocket!)

Right now, we are in the home stretch of the Medicare Advantage Open Enrollment Period. This window closes on March 31.

While PACE enrollment can often happen throughout the year, this is the time of year when many people realize the plan they chose back in December isn't actually meeting their needs. Maybe you realized your co-pays are too high, or you're feeling more isolated than you’d like.
